Accident Summary:

ON ^23 MARCH 1969^, A ^UH-1C, SN 66-15114^, FROM THE ^92D AHC^ HAD THE MISSION OF PROVIDING GUN SUPPORT FOR A COMBAT ASSAULT BEING CONDUCTED BY THE ^53D ARVN RANGER^ OUT OF ^DALAT LIEN KHUONG, RVN^. THE AIRCRAFT, WHICH IS STATIONED AT ^BAO LOC, RVN^, WAS PREFLIGHTED THAT MORNING BY THE AIRCRAFT COMMANDER AND PILOT AND FOUND IN GOOD CONDITION. THE AIRCRAFT DEPARTED THE ^BAO LOC^ AREA ABOUT 1200 HOURS AND ARRIVED 50 MINUTES LATER AT ^LIEN KHUONG^. THE AIRCRAFT WAS SHUT DOWN AND STOOD BY FOR THE COMBAT ASSAULT TO BEGIN. THE ASSAULT STARTED AT 1330 HOURS AND THE AIRCRAFT COMPLETED THE FIRST PORTION OF THE ASSAULT BEFORE DEPARTING FOR FUEL AT ^DATAL CAM-LY AIRFIELD^. AIRCRAFT ^#114^ MADE ITS APPROACH TO RUNWAY 280 AND LANDED AT THE POL POINT. THE AIRCRAFT COMMANDER SUPERVISED THE HOT REFUELING AND INSTRUCTED THE PILOT TO SIGNAL WHEN THE AIRCRAFT HAD 850 TO 900 POUNDS OF FUEL. AFTER COMPLETING THE REFUELING, THE AIRCRAFT COMMANDER CALLED THE CONTROL TOWER FOR CLEARANCE. THE FUEL QUANTITY HAD STABILIZED AT 880 POUNDS. THE AIRCRAFT COMMANDER REFUSED THE INITIAL TAKE OFF CLEARANCE GRANTED BY THE TOWER BECAUSE OF A QUARTERING TAIL WIND. HE GRANTED THE AIRCRAFT COMMANDER PERMISSION TO DEPART RUNWAY 100. THE AIRCRAFT COMMANDER SAID A HOVER CHECK WAS MADE AND THE AIRCRAFT HOVERED EIGHT INCHES TO A FOOT OFF THE GROUND WITH 6300 RPM INDICATED. THE TORQUE AND N1 READINGS WERE NOT MENTIONED. AS THE AIRCRAFT WAS TURNED TOWARDS THE ACTIVE FROM THE POL, THE RP, DROPPED TO 6200. THE CREW CHIEF AND GUNNER GOT OUT OF THE AIRCRAFT AND WALKED TO THE ACTIVE RUNWAY AND WAITED FOR THE AIRCRAFT TO BE HOVERED OUT INTO POSITION FOR TAKEOFF. THE CREW CLIMBED ABOARD. THE AIRCRAFT COMMANDER CALLED THE TOWER, "HOVER CHECK COMPLETE, ON THE GO." A RUNNING TAKE OFF WAS INITIATED AND THE SKIDS REMAINED IN CONTACT WITH THE RUNWAY SURFACE FOR THE FIRST 30 TO 40 FEET. THE AIRSPEED INCREASED AND JUST BEFORE THE AIRCRAFT ACHIEVED TRANSLATION THE RPM READING WAS 6100. AFTER THE AIRCRAFT ENTERED TRANSLATION, THE AIRCRAFT COMMANDER NOSED THE AIRCRAFT OVER AND LOWERED THE COLLECTIVE IN AN ATTEMPT TO BUILD AIRSPEED AND RPM. THE AIRCRAFT BEGAN TO CLIMB AND MAKE A LEFT TURN TOWARDS THE VALLEY WHICH IS LOCATED OFF THE END OF THE AIRFIELD. THE ROTOR RPM BEGAN TO BLEED OFF AND THE AIRCRAFT COMMANDER LOWERED THE NOSE OF THE AIRCRAFT AND FROZE THE PITCH. THE AIRCRAFT WAS BANKED BACK TO THE RIGHT DUE TO A LACK OF SUITABLE FORCED LANDING AREAS IN THE VALLEY. AS THE AIRCRAFT BEGAN TO SETTLE, THE AIRCRAFT COMMANDER SELECTED A LEVEL AREA TO THE RIGHT OF A DITCH OFF THE END OF THE RUNWAY TO SET THE SHIP DOWN ON. THE TERRAIN SLOPED TOWARDS THE VALLEY. THE AIRCRAFT COMMANDER FLARED THE AIRCRAFT 15 FEET ABOVE THE GROUND AND PULLED IN COLLECTIVE PITCH 5 FEET BEFORE TOUCHDOWN. THE AIRCRAFT HIT THE GROUND SKIDS-LEVEL BUT DROPPED OFF INTO THE DITCH AND STARTED TO ROLL. THE AIRCRAFT CAME TO REST UPSIDE DOWN IN THE WIRE WHICH OUTLINES THE AIRFIELD PERIMETER.\\
